How do I identify the social needs of a client?

In Sociants, an individual’s specific needs are organized according to the social determinants of health (SDOH) to which they are associated.

Identifying social needs

1. Within the digital record, go to the tab titled Needs.
2. Click the Identify Needs button.
   
   1. This step will take you to a new page, where you will find the catalog of social needs.
   2. All the specific needs are organized in a card format.
3. To identify a need, use any of the search tools:
   
   1. Search bar. Write the keywords and press the magnifying glass symbol.
   2. SDOH icon menu. Click the icon of the SDOH you are interested in selecting.
   3. (+) button at the bottom of the page. Use the green (+) button, located on the lower right side of the page.
4. Click the Select This button to choose the card with the social need of your client.
   
   1. The needs that were already selected will appear on the right side of the page.
5. Establish the Urgency and Recurrence in which the referral must be attended and press Select.
6. To select additional needs, repeat steps 3-5.
7. If you want to remove a selection, click Remove on the corresponding card.

Once you finish selecting the social needs, press the Submit button.


All set! Now the social needs you selected will appear in the client's file.
