How do I make a referral?

Making a social assistance referral on Sociants is easy! Just follow the following steps:

  1. Identify the social needs of the individual.
  2. Pair the referral with an internal or external program.
  3. Monitor referral status.

    Note: To make referrals through Sociants, you will need to have the Case Manager user role.

     

    Step 1: Identify the individual’s social needs

    1. Go to the Case Management Portal, located in the main menu.
    2. To open the client's record, press the corresponding Details button.
    3. Find the Needs tab in the client’s record.
    4. Identify the individual’s social needs.
    TIPS:
    • Once you select the client’s social need, a list will appear within the Needs tab, where you’ll see each social need and its associated social determinant.
    • Also, you will see two buttons: a green one, to Make Referral, and a red one, to Remove or delete the need.

     

    Step 2: Match the need with a program

    1. Press the green Make Referral button that corresponds to the need you wish to refer.
    2. The Recommend a Need page will automatically open. This is where the matching of needs occurs.
      1. On the left side of the page you will find the referral’s details.
      2. On the right side you will find the search filters and the list of suggested programs.
    3. Complete and validate all of the referral’s details:
      1. Client’s name
      2. Physical address
      3. Social need to refer
      4. Program from which they participate
      5. Urgency
      6. Recurrence
      7. Explanatory note
    4. Use the search filters to choose a program to refer to.
      1. Sociants will provide a list of suggested providers according to the individual’s physical proximity and the capacity of resources and eligibility criteria of a program.
      2. To make a referral between your organization's programs (internal referral), select My Programs from the dropdown list titled Refer To.
      3. To make a referral to another Community Provider (external referral), select Other Programs from the dropdown list titled Refer To.
      4. To make a referral to the organizations in your networks, select your network of interest from the dropdown list titled My Networks.
      5. Use the Distance filter to expand or reduce the radius of miles within which a receiving program is located.
    5. Once you find a program to refer to, click Choose.
    6. Click the Send Referral button, which is located on the left side of the page.

    TIPS:

    • We recommend that you contact the provider by phone or email BEFORE making the referral through Sociants.
    • Also, remember to leave a written note on the referral to communicate any important details to the receiving program's case manager.

    All set! You have successfully made a referral on Sociants.

    Step 3: Monitor the referral’s status

    You can monitor the status of your sent and received referrals from the Referral Management Portal. In addition, you will receive notifications about the status of a referral via email and text messages.
