How do I search for a community provider in the Directory?

The organizations that make up the Directory are arranged alphabetically on the right side of the page, while search filters appear on the left side.

How to find a community provider:

1. You can narrow your search according to the following categories:
   • Zip code
   • Keywords
   • Social determinants
   • Favorites
   • Population
   • Chronic conditions
   • Account status (active or validated)
   • Organization Type
   • Costs of services
   • Characteristics
2. To view the information of the community provider of your interest, click the View Details button on the corresponding card.

How to contact a Community Provider:

You can directly contact an organization from the Directory of Community Providers.

1. Find the Community Provider section on the main menu and press Directory.
2. Use the Directory’s search tool to locate the card of the organization of your interest.
3. To make a call, press the phone icon.
4. To write an email, click the email icon.
5. A window will automatically pop-up from the phone or email application with the contact information of the organization.
