How do I create a consent form on Sociants?

Informed consents are created in the Settings section. After creating your consent forms, they will be available to be administered within each case record.

A consent form is a document where a person is informed of the risks and benefits associated with receiving a service or participating in a particular activity of your organization. Through Sociants’ informed consents, you can capture the electronic consent of your organization's participants for various purposes.

Creating a consent form:

  1. Go to the Consents tab in the Settings section.​
  2. Click Add Consent.
  3. Write the title and content of your consent.
  4. Use the following codes to insert editable fields to your consent:
