How can I follow up on the status of a referral?

Once you send or receive a referral, it is important to maintain effective communication with the other community provider to follow up on the status of the referral.

You can monitor the status of your sent and received referrals from the Referral Management Portal. In addition, you will receive notifications about the status of a referral via email and text messages.

Here we provide some tips on how to contact another community provider to know the status of a referral.


Communication tips regarding a referral’s status:

  1. To monitor the referral's status, you can write a note to  the other program directly from the referral. Click View Notes.
  2. Give a follow-up call to the community provider to find out the status of the referral.
  3. If after a reasonable amount of time you do not receive a response from the provider, you may cancel the referral and try again with a different provider.

How to exchange notes between providers:

  1. Visit the Referral Management Portal.
  2. Use the search filters to locate your referral.
  3. Once you locate the referral, click View Notes to write a message to the other community provider.
  4. Write your message in the bar that says “Leave a note here”.
  5. Click the plus symbol (+) button to save your note.

Note: Notes cannot be edited once they are sent.
