How do I manage my referrals?

When receiving or sending a referral, you can monitor its status from the Referral Management Portal.

In the Referral Management Portal you will see an updated list of all of the active referrals that have been sent and received by your organization.

To locate your referrals according to their status, use the Referrals and Filtered by search filters.

 

Managing my referrals:

  1. When you send or receive a referral through Sociants, you will receive a notification via text message and email.
  2. Access the Sociants portal directly from the login page or use the link provided in the email (referrals received only).
  3. Go to the Referrals section on Sociants’ main menu. At the top of the page you'll see a drop-down list titled Referrals.
  4. Select Received or Sent, depending on the case.
  5. Each referral includes a direct link to the client's record. To access the record, click View Case.
  6. You can exchange notes with the other program directly from the referral to monitor its status. To write a note, press View Notes.
